Evolent Health is a holding company. Through its subsidiaries, Co. supports health systems and physician organizations (providers), as well as health plans (payers) (collectively, partners), to move their business models from fee for service reimbursement to value-based care. Co. manages its operations across two reportable segments as follows: Evolent Health Services, which houses its administrative simplification solution and certain supporting population health infrastructure; and Clinical Solutions, which includes its care management and physician-oriented total cost of care solutions, along with the New Century Health, Vital Decisions and Evolent Care Partners brands.
